AKction2: Aboriginal Kidney Care Together - Improving Outcomes Now [ 4/1/2021 - 3/31/2026 ]

Brief description This project brings together Aboriginal kidney patients and families, health professionals, health services, academics and researchers. Aboriginal peoples' lived experience of kidney disease and kidney care will inform and co-create improvements in care at an interpersonal, service and systems level. Patient journey mapping, effective collaboration between patients and clinical staff, and embedding Indigenous governance mechanisms will be used to improve both clinical and cultural safety.
