Brief description The interface between primary and hospital based care is a quality and safety flashpoint. Our Centre will investigate the utility of a new model of collaborative care at this interface involving upskilled general practitioners, supported by hospital specialists and multidisciplinary teams caring for people with diabetes, heart failure and people receiving palliative care. We will investigate the impact on health outcomes; consumer and health care professional satisfaction; and economic outcomes.
