Brief description A special diet is essential for keeping people with chronic kidney disease well. However inadequate health literacy prevents many people from following this diet correctly. In this project we will redesign the way health services are provided by dietitians to patients with inadequate heath literacy. We will also evaluate whether altering the type of information provided and the method that it is delivered is more effective than the current model of care.
